I wonder what the effect of losing or gaining altitude has on one's bladder.

Since the pressure is exerted over your body equally, it doesn't seem that much different. Similarly, you don't have to pee worse or less when you transition from pool to atmosphere. Now, one has to take into account that the transition causes movement, and this influences the bladder, and the perception of how badly you have to pee.
