I should qualify that and say that I think the bags begin to lose flavor after 1 use and rarely use them more. The pearls and other more dense tea shapes are designed to be used repeatedly, and so for those I do of course will use many more than 5 times.
well, of course it depends on the sort of tea... green teas can handle multiple infusions, so i kind of feel guilty if i don't use them at least twice..
as for caffiene, my understanding is that since it's water soluable it all transfers within about 20 seconds, meaning subsequent infusions would contain none.
This is highly dependent on the quality and type of tea. A good black tea can be used 2-3 times. I can't imagine a white tea being used more than once.
With green tea you're probably looking at one time as well, usually.

My company has started buying the tiny 8-oz. styrofoam cups, so I have to take it out within two minutes or it's way too damn bitter and nasty.
Although I have made big huge 16 oz. cups of chai with two bags, and reused them again later.
And finally, if you want to decaf. your tea, pour two cups of boiling water onto the bag, then toss it, and brew as normal.
